Faculty Appointment Paper Checklist

A. ORIGINAL APPOINTMENT (NEW HIRE)

o Fully Accomplished Basic Paper (HRDO B-1) (1 copy) [obtain at department]
o 1 ½ x 1 ½ Photo pictures [needed for CV and personal data sheet]
o Curriculum Vitae (CV) with 1 ½ x 1 ½ picture (2 copies)
o Justification from chair/director endorsed by dean [obtain at department]
o Transcript of Records (Original) [obtain at alma mater]
          * for new UP graduates, True Copy of Grades (TCG) certified instead
o Diploma (Certified Photocopy)
o NBI Clearance [obtain at NBI – must register online]
o Marriage Contract (NSO Authenticated), if married
o Birth Certificate (NSO Authenticated)
o Drug test [from laboratory testing facilities, takes one day to get results. This is needed for the medical certificate]
o Documentary stamp [from any post office (PHLPOST). This is needed for the medical certificate]
o Community tax certificate (Sedula) [from City Hall, needed for notarized personal data]
o Medical Certificate with documentary stamp (1 copy)
[Get endorsed medical certificate form from department then proceed to HRDO at Quezon Hall.Proceed to UP infirmary to perform free dental check and paid laboratory tests, such as urinalysis, Xray, and fecalysis. Lab tests take a day to get results. Make sure to bring drug test results and ID containing blood type. If not, blood test can be performed in the lab. After a day, obtain and bring lab test results to triage / front desk, which will direct you to the resident doctor for physical exam and release of form.]
