Fieldwork Guidelines for Research
Faculty members in-charge of the fieldwork (including thesis work) must request a permit from the dean (college of science) at least 3-4 days before the trip by completing the forms below and uploading to the CS Online Access Portal. Once approved, upload all documents to UTAK for the chancellor’s approval. MOUs and MOAs are required depending on the nature of the activity.
- Cover Letter to the Chancellor with endorsement of the CS dean and IB director (reviewed by DDRE)
- Special Detail Local Travel (for faculty and UP staff on active duty only)
- Travel permit documents [All UP and non-UP personnel (project-based contractuals such as consultants, field guides, field assistants, etc.) and students must be included in the travel permit documents. Volunteers and secondary joiners without any contract of service or job orders* are NOT permitted to join the field activity.]
- Proof of travel insurance for all individuals joining the field activity
- LGU permit letter and coordination with PNP and the military
- Documents for students and interns joining the field activity (if any)
- Certificate of appearance and travel completion (For travel reimbursements)
- Documents required by CS
* Individuals must be of legal age (at least 18 years old) in order to enter a contract of service or agreements for job orders. Note that some students may not be eligible to enter into a contract of service or job order due to their scholarship guidelines.
Faculty on leave or sabbatical who will be engaged in field activities under the name of the university (e.g. thesis or research) must secure an approval permit from the CS Dean with the relevant attachments indicated above.
